Please submit the information for the name badge.  It can be a MAXIMUM of 3 (Three) lines. Please remember that the more letters the smaller the text. Should you require a sample please request it via email and it will be sent back in 24 hours. 

For Logos please email Black and White camera ready artwork only to clrawards@yahoo.com.
